For a while I wondered if it was just me — if I was being more sensitive than I should, more prone to taking offense.

You, Frank? Never, surely. The issue at hand, of course, is waiter-touching: yay or nay. The answer: pretty much always "nay."

The gesture bothers me more at the beginning of a meal than at the end, because the server is making a set of assumptions about the kind of rapport I and my companions want.

Reasonable enough, we guess: we're not huge fans of human contact ourselves. And we definitely don't want that guy in the picture helping us put our napkin in our lap.
